At EPHC, we understand that the decision to entrust full time care of your loved one to a skilled nursing facility is one of the hardest you’ll ever have to make. That’s why we encourage you to come visit us and see for yourself the high level of medical, compassionate care we offer. The Directors of both our Portola and Loyalton facilities are easy to reach by phone to answer your questions.
A licensed physician supervises all of our residents’ care. We provide 24 hour expert nursing care as well as as assistance with everyday tasks.
We offer physical and occupational therapy. Residents are encouraged to participate in a variety of social activities, such as morning exercises, games, and religious services. They also enjoy a wide range of entertainment, including musicians and dancers, 4-H pets, Christmas carolers, and more in our common room. We have a beauty salon on site, a residents’ garden, and we offer off-site excursions.
Our facilities are small and personal. Residents live in individual or double rooms, and we accommodate a maximum of 27 people at our Portola SNF and 39 at our Loyalton facility. We encourage residents and their families to bring in personal items to decorate their rooms, and we hope that family and friends will visit often. We do everything we can to help our residents feel at home.
Our stand by emergency room, lab, and radiology are just down the hall at our Portola campus, which also offers respiratory therapy. Residents who need these services are spared the physical and emotional strain of having to travel for these vital services.
We accept Medicare and Medi-Cal, as well as most other insurance plans.

In Patient Rehabilitation Services
