Eastern Plumas Health Care’s CalAIM (California Advancing and Innovating Medi-Cal) team is dedicated to helping their members succeed. Providing Enhanced Care Management (ECM) services, they offer a support system assisting community members through the hardest times, utilizing Community Supports and other resources. The team delivers care coordination with a positive, thoughtful, and genuine approach. They listen to what is important to their members and are excited to aid individuals in achieving goals they have tailored themselves.
The CalAIM team serves adults and children in Plumas and Sierra Counties. Qualifying for ECM services requires certain eligibility including being covered by Medi-Cal, which is Partnership HealthPlan of California in Plumas and Sierra Counties. Potential applicants must also meet specific criteria such as having a chronic condition or multiple ER visits, being pregnant or postpartum, or at risk for long term care to name a few.
