Enhanced Care Management
Eastern Plumas Health Care’s CalAIM Community Care Team (California Advancing and Innovating Medi-Cal) provides you with a care manager who can connect you to the healthcare resources and social services you need.
YOUR CARE MANAGER CAN:
- Identify providers and schedule health visits
- Assist with rides to and from health visits
- Connect you to programs and services available in your community
- Help you understand your medications
- Assist with mental health coordination
- Assist with relocation back to the community
- Coordinate treatment for drug and alcohol use
- Support housing insecurities
YOU MUST MEET THESE REQUIREMENTS TO GET ECM:
- Must be a current Partnership member
- Be part of one or more of these groups:
- Adults and children who are at risk for hospital or emergency stays that could be avoided *
- Adults at risk for long-term-care *
- People moving from a nursing home to the community *
- Adults and children with special needs *
- Adults and teens who are pregnant or have a new baby (up to 12 months old) *
- Children and youth involved in Child Welfare *
- Adults and children who have no homes
- Adults and children with severe mental health and/or who want to stop using drugs or alcohol
- Children signed up for California Children's Services or Whole Child Model with extra needs
- Adults and children moving from prison to the community
* These populations are currently served by EPHC CalAIM Community Care. There are additional ECM providers ready to serve populations not managed by EPHC.
