CalAIM Community Supports
Participating in Community Supports is voluntary and there is no charge to the individual. Partnership currently offers the following Community Supports Services in Plumas and Sierra Counties.
- Housing Transition Navigation Services – Assist members find housing
- Housing Deposits – Help members obtain housing such as an apartment or home (deposit, set-up fees/first month utilities)
- Housing Tenancy and Sustaining Services - Help members stay housed including education and training on roles, rights, responsibilities, and how to build key relationships between tenants and landlord/property managers
- Short-term Post-Hospitalization Housing - Provide members with on-going support necessary for recuperation and recovery after discharge from inpatient or ED
- Recuperative Care (Medical Respite) - Allows mem- bers to continue recovery and receive post-dis- charge treatment while obtaining access to primary care, behavioral health, case management, and other supportive social services
- Respite Services - Provided to the caretaker of the member in their home or at another location being used as their home; provided on a short-term basis; distinct from medical respite/recuperative care
- Personal Care and Homemaker Services - Provided to members who need assistance with Activities of Daily Living (ADLs) such as bathing, dressing, toileting, ambulation, or feeding; Personal Care can also include assistance with Instrumental Activities of Daily Living (IADLs) such as meal preparation, grocery shopping, and money management
- Meals/Medically Tailored Meals (MTM) - Provided to members in their home to meet unique dietary needs due to chronic illness; medically supportive food and nutrition services including medically tailored groceries and healthy food vouchers (MTM is not meant to address food disparity).
