Helping You Get to the Care You Need
At Eastern Plumas Health Care, we know that in rural communities, transportation can be one of the biggest barriers to accessing care. Distance, weather, mobility challenges, or lack of reliable transportation should never stand between you and your health.
To better support our patients, we offer Non-Medical Transportation (NMT) and Non-Emergency Medical Transportation (NEMT) services for individuals with scheduled appointments at our locations.
Transportation is available to and from:
- Eastern Plumas Health Care – Portola Campus
- Loyalton Medical Clinic
- Graeagle Medical Clinic
What We Provide
If you are a member of Partnership HealthPlan of California, transportation to medical appointments may be a covered benefit. For eligible Partnership members:
- Non-Medical Transportation (NMT) may be available for routine appointments.
- Non-Emergency Medical Transportation (NEMT) may be covered when medically necessary, including wheelchair transport.
- Authorization may be required depending on the type of transportation needed.
Our Transportation Coordinator will assist in verifying eligibility and obtaining any required authorization before confirming your ride whenever possible.
Eligibility are determined by Partnership guidelines and individual member benefits.
Who Is Eligible?
Transportation may be available for patients who:
- Have a scheduled appointment at one of our EPHC locations
- Do not have reliable transportation
- Have mobility needs requiring wheelchair transport
- Have transportation benefits through their insurance
All rides are subject to availability, medical necessity (when required), and insurance authorization when applicable.
Insurance & Payment Options
Transportation services may be:
- Covered through certain insurance plans when authorized
Coverage varies based on your specific plan and eligibility. Our team will review options with you before scheduling whenever possible.
How It Works
- Submit a transportation request using the form below or call our Transportation Coordinator.
- We review your appointment details and transportation needs.
- If applicable, insurance eligibility and required authorizations are verified.
- Once scheduled, our driver will pick you up and transport you to your appointment.
- Return transportation is arranged as part of the scheduling process.
- On-demand or same-day rides are not available and that all transportation must be pre-scheduled
